A reaction is a special kind of action that can be made outside of your turn. A reaction can only be made as an immediate response to a trigger of some kind, as specified by whichever reaction you intend to use. If your reaction is made during another creature's turn, that creature can continue its turn as normal after your reaction resolves. Once you make any reaction, you can't make another reaction until the start of your next turn.
Everyone can Catch or use a Readied Reaction, but you can make an Opportunity Attack only if you have a trait says you can. Your class, spells, and other effects or circumstances can grant you more options for how to use your reaction. Any special reactions an NPC can take are included in its stat block.
Catch
When sometimes tosses an item your way with their Use action, you can catch it with your reaction.
Opportunity Attack
Not everyone can use this reaction. You might have a feature that says, "You can make opportunity attacks." Some monsters have this feature too, especially those who excel in melee combat.
To make an opportunity attack: When an enemy on its own turn is within 5 feet of you and moves away from you, you can use your reaction to make one melee attack targeting that creature. The attack occurs right before the enemy leaves a 5-foot radius around you. You can't make an opportunity attack if the enemy's movement was caused by teleporting or being Shoved, or if your attack roll has disadvantage.
Readied Reaction
During your turn, you can use the Ready action to state an action you will take when specific criteria are met. If those criteria are met before the start of your next turn, you can use your reaction to take the action you previously stated.
