Rod of Discord (5e Equipment)

Rod, very rare (requires attunement) A short metal rod with a rotund, vaguely humanoid figure made of pink crystal at the end. When attuned to this item, you can use a bonus action to teleport, along with all equipment you are wearing or carrying, to an unoccupied space within 120 feet of you that you can see. Doing so will put you into the Chaos State for 1 minute, which cannot be removed by spells or effects. Chaos State. While in the Chaos State, any effects that cause you to teleport or move into a different plane cause you to take 3d10 necrotic damage that ignores resistance and immunity and cannot be reduced in any way. If this damage reduces you to 0 hit points, you die instantly, as your body materializes mangled and rearranged. The Chaos State is inflicted as a result of all teleportation or plane-shifting spells or effects, not just the Rod of Discord itself.
