Diabolus ex Nihilo (5e Equipment)
From D&D Wiki

Weapon (longsword), artifact (requires attunement)
This dark and deathly item has fueled the insanity of powerful men past. Those same holders of the blade were heads of mass murder, slaughtering their victims for blood. It is said this was erected from a single bone of Lucifer dipped in his blood by his followers after Asmodeus deposed him. As such, it bears all his malice.
Nature can't abide the unsheathed blade's presence. Ordinary plants wither in its presence, animals are unwilling to approach it, and the blade gradually destroys whatever it touches. Even stone cracks and turns to powder if the blade rests on it long enough.
You gain a +3 bonus to attack rolls and damage rolls with this magic weapon. Your critical hit range while wielding this sword is 18-20.
Diabolic Sentience. This weapon has a telepathic bond with whoever wields or carries it. It speaks to their mind as demonic whispers that can be distracting or interpreted as gibberish. If asked a question, it is prone to lie and suggest the worse things to happen or the most detrimental choice to its user.
Slaying Blade. The sword has the properties of a vorpal sword. A critical hit will also use this property.
Vampiric Hilt. The hilt of the sword is engraved to resemble vampire fangs and stores energy from those whom it slays. For every Medium or larger creature killed, the sword gains 1 charge. The creatures cannot be constructs or undead. You may expend one charge to cast vampiric touch without material or somatic components.
Diabolical Avenger. When you hit a living being with it, that creature takes an extra 2d10 necrotic and fire damage.whenever you use this weapon to reduce a creature to 0 hit points, the sword slays the creature and banishes its soul to the Void, unless it is a construct or an undead. A creature whose soul has been banished by Diabolus ex Nihilo can be restored to life only by a wish or true resurrection being cast on the creature's original body.
Aura of Fear. While you hold the drawn sword, it creates an aura of fear in a 15-foot radius around you. When a creature enters the aura for the first time, they must make a DC 18 Wisdom saving throw. On failure, they become frightened of you for one minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If the creature succeeds then it is immune to this effect for the next 24 hours.
Insanity. When you kill a creature with this weapon, you must succeed a DC15 Wisdom saving throw or be inflicted with short term madness. If you already have short term madness, you are inflicted with long term madness. If you have long term madness, you are then inflicted with indefinite madness.
Mark of the Apocalypse. While you hold this weapon you are immune to radiant and necrotic damage. Also, you gain a advantage on saving throws against spells and all magical effects.

Destroying the Diabolus ex Nihilo. The only way to destroy this weapon is to baptist it in the holy water from the High Heavens, and have it be prayed on by a group of high level clerics (minimum of 15) for 30 days and 30 nights. Also it must be consecrated in the blood of a powerful Solar. If this is not done correctly, then the blade will shatter and spread across the world. If the shards are found and gathered, the sword magically reforges and reforms itself.
