Weapon (Power Scythe), legendary (requires attunement by a champion of a plague/death god)

The Manreaper is a weapon of pure suffering it does not promise a quick death. It was once a normal power scythe, but has been seeped in disgusting juices of the most virulent plagues and indescribable filth of a thousand realms, these are only given to the most fervent followers of dark gods.

Filth-Soaked blade. This is a pretty sick blade, I'll see myself out. On a hit, this weapon deals an additional 3d8 poison damage.

Harbinger of Plague The blade has been covered in the most virulent diseases known to life. When a creature is hit by this weapon, they must make a DC 18 Constitution saving throw, becoming infected with a disease of the DM's choosing on a failure.

Rot-Wood Handle The wood used in the handle is magically conductive, allowing this weapon to be used as an arcane focus
