Random Properties. The Perfected Oculus has the following random properties:

  • 1 minor detrimental property
  • 1 major detrimental property


The Perfected Oculus is a masterfully made mechanical eye, constructed from mithril, adamantine, and precious gemstone. To attune to it, you must gouge out your own eye and press it into your empty socket. Once in place, the eye wires itself directly into your brain and cannot be removed.

Perfected Vision. If you are attuned to the Perfected Oculus, you gain the following benefits:

  • You have truesight with no range limit.
  • You are immune to being blinded.
  • You can make out details of even extremely distant creatures and objects as small as 6 inches across. Because of your truesight, you are not limited by visibility or weather conditions; the only obstacle is the horizon.
  • You can see anything within 1 foot of you minutely, as if looking through a magnifying glass or a microscope. Within that range, you can focus enough to make out the atomic structure of objects or creatures.
  • You can see into and through solid matter. The vision can penetrate 10 feet of stone, 10 inches of common metal, or up to 30 feet of wood or dirt. Thicker substances block the vision, as does a thin sheet of lead.
  • You can see an aura around creatures or objects bearing magic within your sight, and can tell its school of magic at a glance (if any).
  • You can identify poisons, poisonous creatures, and diseases within your sight, as well as their exact kind.
  • You can identify aberrations, celestials, elementals, fey, fiends, and undead within your sight, as well as places or objects that have been magically consecrated or desecrated.
  • You have a +10 bonus on Wisdom (Perception) or Intelligence (Investigation) checks that rely on sight, as well as to your passive Perception.
  • You have a +2 bonus to ranged attack rolls, and you do not have disadvantage from attacking at long range.
    Destroying the Perfected Oculus. The Perfected Oculus is indestructible while it is attuned to a creature. While is not attuned to a creature, it can be broken with enough effort. The eye is immune to all damage except for bludgeoning damage that is also adamantine, and it has a damage threshold of 100. So to break it, you must inflict 100 points of damage on it with a single blow from an adamantine bludgeoning weapon.
