Neckronomicon (5e Equipment)
From D&D Wiki
magical item (spellbook), artifact (requires attunement)
the neckronomicon is frequently confused with its much more powerful cousin, the necronomicon, mostly because they are identical. this book is far less useful. it was once the prized posession of a long forgotten kingdom, who associated the length of ones neck to be a sign of nobility.
neck production: when you are attuned to the book, your neck takes an oily sheen. if you are decapitated, your head will shrivel up and fall off, and a new one will grow in its place. you regain any hit points lost from the beheading, and there are no ill effects.
