Man-eating Mushrooms (5e Hazard)
From D&D Wiki
Man-eating Mushrooms[edit]
Man-eating mushrooms, aside from their purple coloration and rather large size, appear to be your average garden variety mushrooms.
To properly identify the mushrooms, one must make a DC 12 Nature check to notice the dangerous nature of these fungi.
When a creature, person, or any living thing of medium or smaller size comes within 5 feet of it, the mushroom will quickly swallow them whole, keeping them tucked away in their cap. To avoid being swallowed, one must make a dexterity saving throw (DC 15). If one fails to avoid the mushrooms jaws, then they will slowly be marinated in a lethal poison in the mushroom's digestive system, which has a side affect similar to that of a hallucinogenic drug, giving the victim a slight buzz and making them docile. Another person may attempt to cut the mushroom open to remove the person inside, but must do so within approximately 5 minutes or else the person inside will be dead from the digestion process. Cutting down the mushroom awards the person who slew it 10 experience.
