Falling Cross Shaped Rock (5e Trap)
From D&D Wiki
Falling Cross Shaped Rock[edit]
A tile in the floor that when stepped on releases a large portion of the building's stone ceiling in the shape of a cross. In order to avoid the trap, one must either identify the tile with a DC 15 Perception check and avoid it. Should they activate it, the creature must make a DC 15 Dexterity saving throw to avoid the falling object. On a success, the rock is narrowly avoided and the victim escapes unscathed.
In the event of a failure, the victim collapses to their knees and is crushed by the weight of the rock, dealing 10d20 Bludgeoning damage and rendering the creature restrained and prone. The creature can break free of the restraint with a successful DC(20) Strength check.
