Aboleth Choker (5e Equipment)

Wondrous Item (choker), very rare (requires attunement)

This choker seems to resemble an aboleths tentacle.

When put on the wearer that is attuned, they must succeed a DC 20 Constitution saving throw or the choker will come to life and sink into their neck the two small tentacles will sprout out of the back of their neck and wrap around their neck connecting at the front. After 1 day the wearer's teeth will turn into sharp fangs and they will sprout 4 prehensile tentacles each of which can lift up 5lbs from their back that can easily be hidden. As a bonus action on your turn, you may attack a creature within 5 feet of you with these tentacles. Upon hitting they deal 2d8 + your Strength modifier magical piercing damage.
