Incorporeal. The xenogog can move through other creatures and objects as if they were difficult terrain.

Lightning Absorption. Whenever the xenogog is subjected to lightning damage, it takes no damage and regains a number of hit points equal to the lightning damage dealt; if this is 20 or more, the xenogog's Multiattack is also recharged.

Radiation. For each cumulative minute a creature spends within 10 feet of any xenogog, it takes 6 (1d10 + 1) radiant damage. Additionally, after each cumulative hour in this area, a creature must succeed on a DC 19 Constitution saving throw or gain a level of exhaustion.

ACTIONS

Multiattack (Recharge 5-6). The xenogog makes three electroshock attacks.

Electroshock. Melee or Ranged Weapon Attack: +10 to hit, reach 5 ft. or range 30/60 ft., one target. Hit: 52 (8d12) lightning damage.

Endocauterize. The xenogog targets one creature it can see within 5 feet, attempting to reach into its body and sear it from the inside. The target must make a DC 18 Dexterity saving throw, taking 37 (5d6 + 30) fire damage on a failed save or half as much on a success. If the save fails by 5 or more and the target isn't a construct or undead, the xenogog may choose to destroy a portion of the crature's brain or equivalent organ; when it does so, the target is stunned for 1 minute, its Intelligence and Charisma scores become 1, and it can't cast spells, activate magic items, understand language, or communicate in any intelligible way. The creature can, however, identify its friends, follow them, and even protect them. This effect can be ended by greater restoration, heal, or wish.

A xenogog is a humanoid figure under a meter in height, resembling an artificial suit comprised of a slick and flexible tissue with the consistency of tough rubber; the cranium is roughly spherical, bearing a single concave pit of pulsating, concentric red rings. As this pit is turned toward external stimuli, it can be deduced to perform a broad sensory function, and visual contact with it has been known to induce minor dizziness, tinnitus and dry mouth. Their internal anatomy appears to consist largely of variously colored, plastic-like tubing in a complex arrangement, but they possess no other tissues, organs or fluids. They are often accompanied by a buzzing sound, and their odor is likened to burnt keratin. They are usually unrestricted by gravity but locomote somewhat slowly, as if through a fluid environment, and their bodies selectively ignore most physical matter. Xenogog are passively radioactive, and require only electricity to sustain themselves, which they may also discharge in a potentially hazardous burst if needed.
Xenogog's arms each conceal a thin, telescopic needle of an unknown metallic substance in a nearly permanent state of intangibility. The entity can solidify the tips of these appendages for only a millisecond at a time, during which they emit a temperature of 1300 degrees C. By this mechanism a xenogog may reach inside solid materials and inflict a targeted burn, a method by which it will attack the brains of other organisms with surgical precision.
Xenogog typically show a brief hesitation to interact with reflective surfaces, as though ascertaining an unknown danger before contact. They are difficult to communicate with in detail, expressing comprehensive thoughts only briefly and uncommonly. The occasional complete, articulate sentence demonstrates a highly adaptable intellect, but remains seemingly unable or unwilling to further clarify its nature. Very rarely a specimen has expressed the concept of "back" before attracting a bolt of lightning and subsequently de-manifesting.
