I like this. Tidy and concise. But I'm not seeing any mention of will saving throws. Does that mean a phobia can't be overcome?

I think that being shaken should be the minimal penalty for being in the presence of the trigger, with no chance of bypassing it. If you have a moderate DC, the penalty will not happen frequently enough for someone with an optimal Will save (so they get a free feat). If you raise the DC to counter this, you may as well not have it at all for normal players.
However, if you feel a Will save belongs somewhere, it could be tied to avoiding a more serious panicked or frightened condition. But still, what DC? If it's static, it becomes easy to bypass at higher levels, but adding in a "+1/2 HD" seems contrived. I would favour a static DC - a hero should be able to overcome his fears with experience - and they are still at least shaken. Marasmusine (talk) 13:49, 6 June 2013 (MDT)
