Suggestions to improve clarity:
I believe "succeed on a d10 Perception roll" should read instead "succeed on a DC 10 Perception check."
Instead of "On a success of 15 or higher,...," perhaps use this: "On a Perception check roll of 15 or higher,...."
Instead of "If they succeed on a 14 or lower,...," perhaps use this: "On a Perception check roll of 10 to 14,...."
Instead of "If a character or object of equal size to the patch or smaller passes through the grass, they are teleported to another patch of Blinkweed within 1 mile, given that there are any and are not covered or blocked. " perhaps use this: "A character or object entering a patch of Blinkweed is teleported to any other existing (randomly-selected) patch within 1 mile." Also add this sentence: "Anything teleported to a 'new' patch may remain in, move within, or exit the new patch without being teleported again; however, if the new patch is re-entered, teleportation to another randomly-selected patch occurs."
I think it's unnecessary to say "...not covered or blocked..." because Blinkweed is a weed, and as such, it would grow only in locations where it is uncovered and has access to sunshine and rainfall. [If someone teleports a pallet of bricks onto a patch of Blinkweed, the patch will die and, thus, will no longer be a patch of Blinkweed.]
Suggestions to improve balance:
The following feature: "The Blinkweed can be harvested, and if consumed raw, will grant the ability to see 10 feet into the Etherial [need to correct spelling to Ethereal] for 1 hour" is very powerful. I would suggest either deleting it or adding some type of "cost" to balance it. For example, perhaps for the time that you see into the Ethereal plane, you are also blind in the Material plane.
I would suggest deleting the remainder of the entry from the point, "If tossed in a fire,...," because the additional properties make Blinkweed too powerful. However, these are very interesting concepts. Perhaps, instead, make a second type of weed, perhaps named "Smokeweed" or "Fireweed," that has the properties described in the remainder of the Blinkweed entry.
