The snake lord apostle in its [humanoid] and [apostle] forms in Berserk chapter 0C.

The human that would become the slug count was the count of a small hold overrun by heretics. After he was certain his land had been purified, he returned only to see his beloved wife willingly participating in a heretical orgy. Killing the party in blind rage, he came face to face with his wife, who only smiled triumphantly in his unwillingness to kill her. Moments before he would have committed suicide, a behelit he had on his person for decades awakened, allowing him to sacrifice her to the God Hand and become an apostle. He continued to hunt "heretics", though many of his new targets were innocent, with enhanced zeal. Those that were captured were tortured, mutilated, and consumed by the count himself.
