Wendigos are monstrous creatures born from humans with insatiable greed, inevitably ending in the consumption of human flesh. In their descent into madness, their skin is pulled taut against their bones as the pangs of hunger set in. The hair on their head falls out as coarse hair covers their body. As the transformation progresses, flesh begins to slough off of bone, revealing a deer-like skull with jagged teeth filling its maw. When the transformation is complete, no humanity remains; only hunger.

The Skull of the Wendigo is a deer-like skull with large antlers atop its crown. Any creature who touches this skull must make a DC 18 Wisdom saving throw or be compelled to place the skull on their head, as the whispers of madness fills their mind. When you place this skull on your head, its size and shape change to fit you. While wearing the skull, you gain the following abilities:

  • Darkvision. You can see in dim light within 60 feet of you as if it were bright light, and in darkness as if it were dim light. You can't discern color in darkness, only shades of gray.
  • Powerful Build. You count as one size larger when determining your carrying capacity and the weight you can push, drag, or lift.
  • Inhuman Agility. Your movement speed increases by 15 feet. While in dim light or darkness, this bonus increases to 30 feet. Additionally, you can move across any liquid surface as if it were harmless solid ground.
  • Keen Senses. You have advantage on Wisdom (Perception) checks.
  • Rend and Tear. Your bite and claws are natural weapons, which you can use to make unarmed strikes. If you hit with your bite, you deal piercing damage equal to 1d8 + your Strength modifier, and if you hit with your claws, you deal slashing damage equal to 1d6 + your Strength modifier, instead of the bludgeoning damage normal for an unarmed strike. When you use the Attack action with your bite, you can make one unarmed strike with your claws as a bonus action.
  • Monstrous Resistance. You have resistance to bludgeoning, piercing, and slashing damage from nonmagical attacks. In addition, you have advantage on saving throws against spells and other magical effects.
  • Cannibalize. When you hit a humanoid with your bite attack, you regain a number of hit points equal to the damage dealt.
  • Wendigo Psychosis. You must make a DC 18 Wisdom saving throw whenever you take a long rest while attuned to this item. On a success, you rest normally. On a failure, you experience vivid lucid dreams in which you transform into a blood-starved beast wreaking havoc across the land. You gain none of the benefits of taking a long rest, and may be subject to exhaustion. With each successive failure, the dreams last longer and become more surreal and gruesome. After a total of five cumulative failures, you awake to find the skull affixed to your head, and any attempt to remove it is met with failure. After you have succumbed to wendigo psychosis, you gain the Everlasting Hunger feature, and you no longer make saving throws when taking a long rest.
    • Everlasting Hunger. You must consume at least one humanoid corpse each day in order to gain the benefits of taking a long rest. Consuming a corpse takes 1 hour, and can be completed as part of a short or long rest. You gain a number of temporary hit points equal to your level whenever you consume a corpse.

Once you have attuned to this item, only the effects of a greater restoration spell or similar effect may break your attunement. Once you have succumbed to wendigo psychosis, nothing short of a wish spell is capable of removing the curse.
