Release From Torment. When the skull is reduced to 0 hit points, it explodes into flames, and it dies. Every creature within 20 feet of it must make a DC 15 Dexterity saving throw, taking 11 (2d10) fire damage plus 11 (2d10) psionic damage and is considered under the effects of mindshriek on a failed save, or half as much on a success and not under the effect of mindshriek.

ACTIONS

Slam. Melee Weapon Attack: +3 to hit, reach 5 ft., one target. Hit: 4 (1d6 + 1) bludgeoning damage plus 4 (2d4) fire damage.

Nether skulls are created through a dark ritual which requires the extensive torture of the victim. The skull is eventually removed from the body with the tortured soul remaining trapped inside. At this point, necromancers will often store these skulls in the Nether Realm, where they continue to suffer until called upon by name to the mortal realms to serve their dark masters.

Undead Nature. A nether skull does not require air, food, drink, or sleep.
