Leed's Devil (4e Template)
From D&D Wiki
Those that fall victim to the Leed's Curse find themselves transforming into a Leed's Devil. As a victim falls to the curse their body is twisted and reshaped into a hideous amalgam of various animals. They stand about eight and half feet high, with a head like a dog and a face like a horse. They have a long thick horse-like neck, bat-like wings about two feet long, and back legs like those of a goat with horse's hooves. The devil stands upright on its back legs with two short front legs ending in clawed hands. The victim's alignment is changed to Chaotic Evil and they become filled with a cannabalistic bloodlust, driven to kill any creature that strays too close to it or the item it protects. When satiated the devil is driven to protect and hide away with whatever object bore the curse that transformed them. With thier digestion slowed devil's often hide for decades before needing to come out and feed on any humanoids it can find in its area. Then once full it returns into hiding with its "prize". Once transformed the curse can be lifted and the victim restored with an appropriate level Remove Affliction ritual.
