Blood Root (5e Hazard)
From D&D Wiki
Blood Root[edit]
To the casual observer, blood root first appears to be a red flower, grass, or similar common plant. However, a creature who succeeds on a Perception or Nature check (DC 10) will notice the dangerous, exposed red roots around the plant base.
An unaware creature will be restrained by the blood root if they come within 5 feet of it, unless they succeed on a DC 13 Dexterity saving throw. Upon failure, a protruding red root will grab their leg and they will take 2d6 piercing damage with an additional d4 piercing damage at the start of each of their following turns until they can succeed on a Constitution saving throw (DC 10).
Dealing 3 slashing damage (AC 5) will cause the root to die.
