Sourtrout (5e Creature)
From D&D Wiki
Sourtrout
Large monstrosity, any chaotic Armor Class 15 (natural armor)
Saving Throws Charisma +2 Tetrodotoxin. Whenever the sourtrout makes a successful bite or skewer attack against a creature, the target must succeed on a DC 12 Constitution saving throw or become paralyzed and immediately begin suffocating. Creatures paralyzed this way can remake the save at the end of their turn, ending the effect on a success. ACTIONSMultiattack. The sourtrout makes four skewer attacks and either a bite or fish breath attack. It can replace two skewer attacks with one grab. Bite. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 21 (3d10 + 5) piercing damage.. Fish Breath (Recharge 5-6). The sourtrout exhales a 10-foot cone of rancid miasma. Each creature in this area makes a DC 12 Constitution save, taking 9 (2d6 + 2) poison damage and becoming poisoned for 1 round on a failed save, or taking half as much damage on a success. Fish Head Bomb. The sourtrout discards its head, losing the ability to attack with its bite or fish breath. At the start of its next turn, the head swells dramatically and explodes; each creature in a 15 foot radius makes a DC 15 Dexterity saving throw, taking 12 (2d8 + 3) poison damage plus 12 (2d8 + 3) thunder damage and becoming poisoned for 1 round on a failed save, or taking half as much damage on a success. The head regrows after the sourtrout completes a long rest. Grab. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 10 (3d4 + 3) bludgeoning damage, and the target is grappled (escape DC 14). Until this grapple ends, the target is restrained, and the sourtrout can't grab another target. Skewer. Melee Weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 6 (1d10 + 1) piercing damage.

An extremely rare result of the diamrem breeding process, the sourtrout appears to be the ultimate hunter-killer of its peculiar family.
A sourtrout begins its life as an almost natural looking pisciform, albeit with a five-fingered humanoid hand in place of a tail fin. Over the course of a few weeks the creature grows in length, sinuous and eel like, and already begins to show the patterns of decay associated with related monsters. Soon a pair of copepod parasites emerge from within its cranium, it beaches itself where convenient and it appears to expire, most of its body rotting away at a highly accelerated rate. Within only a day, the monster is devoid of almost all external tissues from its gills down, but it has only then reached what can be considered maturation. Its parasites, anchored deeply in its ossified brain, take over as the true seats of its consciousness, and doubly function as a pair of highly sensitive new eyes. Variant: Dead FishSometimes sourtrout can be created by necromantic magic, or rise from the dead through other means. These sourtrout have the undead type, are vulnerable to radiant damage, replace half of any poison damage they deal with an equal amount of necrotic damage, and gain the following trait: |
