Wondrous Item (Mask), legendary (requires attunement)

The Mask of Indeterminate Faces is a highly durable glass mask with no discernible features that shifts and morphs to fit the users face. While wearing it, it clouds over the appearance of your face so that no one is able to discern who is under the mask. You can don and doff the mask as a bonus action. While wearing the mask, you can innately cast shapechange requiring no concentration and no material components. When you remove the mask, you revert back to your normal appearance.

Curse. After 1d6 plus 1 days after attuning to the mask, the mask becomes adhered to your face and you are unable to remove it by any natural means. Every long or short rest, the mask will forcefully cast alter self on you, changing your appearance until the spell wears off. Every time this happens, you must make a DC 14 Wisdom saving throw. On a failure, you lose lose 1d4 points from your Charisma stat as you slowly forget your true appearance and identity. The only way to remove the mask and end the effect, restoring all stat points in the process if by casting greater restoration whereas the mask will release itself from your face with its magical properties intact. If your Charisma stat reaches 0, the mask releases itself from your face, now having completely lost your identity. Instead of reverting to your normal form, you take a completely new form. You appear as a medium humanoid with pure white skin. You have no physically defining features which includes no mouth, no hair, no eyes, no nose, no ears, openings in your body and no gender specific features. You can still hear as normal even without ears, you no longer need to eat and drink as you are also physically unable to eat and drink, you gain blindsight of 60ft but you are blind beyond this radius, and you no longer require sleep. Instead you must enter into a motionless trance for 4 hours a day. While in this state you do not even dream. After resting in this way, you gain the same benefit that a human does from 8 hours of sleep.

You can be restored by a wish spell.
