The penanggalan is a female humanoid who has been cursed through black magic or demonic forces. During the day they appear to be normal members of their race. At night, a penanggalan's head end entrails detach from her body, compelled to seek living blood and flesh to sustain their lifeforce.
In her humanoid form, a penanggalan avoids eye contact but licks her lips in the presence of blood. Unlike a vampire, a penanggalan is a living person in this form, and such behaviour can be the only way to identify her for what she is. Only when she detaches her head is she considered an undead creature.

Creating a Penanggalan[edit]
- Ability Scores
Your most powerful attacks are made while your head and entrails are detached. They require unnatural speed and agility, so Dexterity should be your highest score. As you are not proficient with heavier armors, a high dexterity is also important for dodging attacks.
The attacks you make whilst in your humanoid form use your force of personality and psychic strength, so your next highest score should be Charisma. Charisma also improves the damage you inflict with all your penanggalan powers, and your regeneration whilst in maiden form.
- Vampire
A penanggalan is a kind of vampire. When you gain a Penanggalan power, you can instead take a Vampire power of the same level and type. The Vampire class is presented in Heroes of Shadow.

Level 1: Blood is Life[edit]
Your undead nature and your dependence on blood force you to use healing surges in a different way than other characters. The few healing surges
you have can be augmented by the life force you drain from your foes with many oryonr powers. In addition, you can draw on the life energy of a willing
ally to fuel your own Vitality.
Benefit: Once during a short rest, an ally adjacent to you can willingly lose one healing surge to allow you to regain hit points equal to your bloodied value. You regain hit points equal to your bloodied value. If you end a short rest with more healing surges than your usual number of healing surges for the day, you lose any healing surges beyond that number but regain all your hit points.
Level 1: Penanggalan Agility[edit]
Your curse and your hunger has heightened your reflexes, making it harder for foes to hit you.
Benefit: While you are wearing cloth armor or no armor, you gain a +1 shield bonus to AC and Reflex.
Level 1: Hidden Might[edit]
You burn with an inner strength that belies your delicate appearance.
Benefit: You gain a bonus to the damage rolls of your vampire powers and vampire paragon path powers. The bonus equals your Charisma modifier.
The bonus increases to 2 + your Charisma modifier at 5th level, 4 + your Charisma modifier at 15th level, and 6 + your Charisma modifier at 25th level.
Level 1: Maiden Form, Head Form[edit]
Certain penanggalan powers will allow you to transform between two types of creature known as "maiden form" and "head form". Some of your powers will gain a bonus depending on which form you are in.
Maiden Form[edit]
When you are in maiden form, you look like a normal female member of your race.
Benefits: You gain the following benefits and weaknesses.
- You are a living creature.
- You gain a +5 bonus to Bluff checks to disguise the fact that you are a penanggalan.
- While you are bloodied, you gain regeneration equal to your Charisma modifier.
Head Form[edit]
In head form, your head and internal organs have detached from your body.
Benefits: While you are in head form, your headless body is considered a corpse. Unless the body is utterly destroyed, you can take a standard action to return to maiden form, but only if it is in the same square as at least one part of the body.
While you are in head form, you gain the following benefits and weaknesses.
- You are an undead creature. You are unaffected by anything that affects only living creatures. You do not need to breathe.
- You have darkvision.
- You have resist 5 necrotic.
- You have vulnerable 5 radiant.
- Your speed becomes 2, and you gain a fly speed of 7 (hover, altitude limit 1)
- You no longer have hands, and you drop anything you were holding, except implements you can use. Although your other equipment remains with your body, you are still linked to your magical items and continue to receive their benefits. The exceptions are magical weapons and wondrous items: You cannot use the powers and properties of those items.
- You cannot use powers with the Weapon keyword.
