Balance On Item Bonuses[edit]
When an item that just has bonuses to attack rolls, spell attacks, saving throws, ect. the item's rarity is determined by two factors. Whether the item has to be held to gain the bonus and the bonus itself. An item which gives a +1 bonus to something is at least uncommon, an item which gives a +2 bonus to something is at least rare, an item which gives a +3 bonus to something is at least very rare, and an item which gives a +4 bonus to something is at least legendary. If the item doesn't need to be held, the item is much better and the item is considered to be 1 rarity higher than usual (+1 bonus would be rare/+2 very rare/+3 legendary), see the Looking at Balance section of the Equipment Design (5e Guideline).
Based on these factors, since the witch's head needs to be held to gain its effect and it provides an average of 2.5 (1d4) to its respective rolls, the item would be of very rare rarity.--Blobby383b (talk) 15:02, 20 February 2020 (MST)
