Spell slot level[edit]
Thank you for creating this table, it is so much better than the original one. May I suggest a variation though: I'd love to take the spell slot level into account. But instead of the level affecting the change for a wild magic surge, I would use it to select the column in the table. Maybe this would work: Per default, the effect is a Nuisance. You roll a d20 and if the result is equal or below say 6 + [the spell slot level] it gets upgraded to Moderate. If it does, roll another d20 and if the result is equal or below 2 + [the spell slot level] it gets upgraded to Extreme. High level spell having more severe surges feels adequate for me. Also it gives low level players less Extreme results for which they may not be prepared for and high level players more Extreme results.
-- Thank you for the feedback! This is certainly an interesting idea, and has merit. My push-back is this already adds mechanics to the game over and above the existing process, and in the interest of keeping 5e streamlined, I'd try to keep the math out of it as much as possible. Since the severe effects are scaled against the caster's spell DC, I think this may already be incorporated to an extent. I've made numerous edits and clarifications since this was posted several years ago.
