We can get an idea of each condition's appropriate tier of introduction by looking at each class's powers and noting the earliest level that the condition appears.
Blinded - Heroic (Cleric 7, Fighter 23, Paladin 19, Rogue 1, Wizard 13)
Deafened - Heroic (No instances in PHB1 powers, but it's no more powerful than Blinded)
Dominated - No instances in PHB 1 powers.
Immobilized - Heroic(Cleric 7, Fighter 5, Paladin 13, Ranger 13, Rogue 17, Warlock 1, Wizard 3)
Petrified - No instances in PHB 1 powers.
Prone - Heroic (Cleric 3, Fighter 1, Paladin 7, Ranger 7, Rogue, Warlock 29, Warlord 9, Wizard 1)
Restrained - Paragon, rarely (Fighter 23, Rogue 17)
Slowed - Heroic (Cleric 15, Fighter 1, Ranger 1, Rogue 1, Wizard 1)
Stunned - Paragon (Cleric 25, Fighter 13, Paladin 27, Ranger 15, Rogue 13, Warlock 25)
Unconscious - Heroic, rarely (Rogue 9, Wizard 1)
Weakened - Heroic (Cleric 1, Paladin 9, Ranger 1, Rogue 25, Warlock 13, Warlord 23, Wizard 1)
