Don't forget that the majority of the Barbarian's most vital combat abilities (Rage Damage Bonus and Reckless Attack) are centred around using Strength-based weapons, so using your Constitution Mod for Attack and Damage Rolls basically defeats the purpose of those Features. This could be fixed by a simple sentence saying something such as: "You are considered Proficient with Blood Weaponry and you can use your Constitution Modifier in place of Strength for the Attack and Damage Rolls, however Blood Weaponry still counts as a Weapon that uses Strength for Attack and Damage Rolls when using Features such as your Rage Damage Bonus and Reckless Attack."

Another thing about Sanguine Presence that doesn't make it that good is the fact that it takes a Bonus Action to fabricate, it deals Damage to you when you make it, and it doesn't have any good Features tied to it until you reach 6th Level (apart from the Str to Con Mod for Attacks and Damage, but even then a Barbarian would most likely have those scores tied in terms of bonuses). Maybe you can incorporate Bloodlust into the 3rd Level Feature from the very start, and have the amount of additional Necrotic Damage go up at certain intervals (1d6 at 3rd, 2d6 at 6th, 3d6 at 10th, and 4d6 at 14th. Or it could scale the same way that some of the Storm Herald's Features scale, which would be 1d6 at 3rd, 2d6 at 5th, 3d6 at 10th, 4d6 at 15th, and 5d6 at 20th). Or you could lose the restriction on the once per Turn additional Damage and just have each successful attack with the blood weapon do extra Necrotic Damage equal to half your Barbarian Level.

Of course, doing this would take some of the power away from the actual Bloodlust Feature, so you could change it and make it something like: "You can choose to drain the blood of your enemy when you Rage. When you do so, you regain Hit Points whenever you deal Necrotic Damage with your Blood Weaponry equal to the amount of Necrotic Damage dealt. You can use this Ability once per Rage."
