Death Burst. When the creeper dies, it explodes in a burst of acid. Each creature within 5 feet of it must succeed on a DC 13 Dexterity saving throw, taking 7 (2d6) acid damage on a failed save, or half as much damage on a successful one.

Claw. Melee Weapon Attack +6 to hit, reach 5 ft., one target. 13 (3d4 + 6) bludgeoning or slashing damage (creeper's choice).

Bite. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. 5 (1d6 + 6) piercing damage.

Spewing Madness. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. 9 (1d8 + 5) acid damage. If a humanoid is reduced to 0 hit points by this attack, it rises as a creeper 24 hours later unless it is restored to life or its body is destroyed.

Bile Throw. Ranged Weapon Attack: +4 to hit, range 15/30 ft., one target. 4 (1d6 + 2) acid damage. If a humanoid is reduced to 0 hit points by this attack, it rises as a creeper 24 hours later unless it is restored to life or its body is destroyed.


Plague creepers are unrelenting infection on any population with the on only desire being to spread the infection before they burn out in 5d10 days from the illness turning them into walking bile bombs.
They are bubbling walking humanoids with seeping black ooze poring from eye sockets and mouth as well as any other injuries they suffer. They often resort to swarm tactics for taking on Larger foes or weaker beings if within reach these beings should be killed as soon as spotted as a infection into a major water source can kill populations

Template

Reduce the creature's Intelligence score to 2 and increase its Strength and Constitution scores by 4 each and add Spewing madness/Bile throw to the creature's attacks.
