Armor Class 18 (natural armor)
Saving Throws Int +9, Wis +8, Cha +8
Magic Resistance. The neothelid has advantage on saving throws against spells and other magical effects.
Spellcasting. The neothelid is a 10th-level spellcaster. Its spellcasting ability is Intelligence (save DC 17, +9 to hit with spell attacks). The neothelid has the following wizard spells prepared:
Cantrips (at will): blade ward, mage hand, poison spray, ray of frost
Multiattack. The neothelid makes three attacks: two with its bite and one with its tail. It can use its tentacle attack in place of its bite.
Bite. Melee Weapon Attack: +12 to hit, reach 10 ft., one target. Hit: 18 (2d10 + 7) piercing damage. If the target is Huge or smaller, it is grappled (escape DC 17). Until this grapple ends, the target is restrained. The neothelid has four tentacles, each of which can grapple one target.
Tail. Melee Weapon Attack: +12 to hit, reach 20 ft., one target. Hit: 20 (3d8 + 7) bludgeoning damage.
Tentacle. Melee Weapon Attack: +12 to hit, reach 5 ft., one target grappled by the neothelid. Hit: 24 (7d6) psychic damage.
Psychic Crush (Recharge 6). The neothelid emits psychic energy in a 60-foot cone. Each creature in that area must succeed on a DC 17 Intelligence saving throw or take 54 (12d8) psychic damage and be stunned for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
Acid Breath (Recharge 6). The neothelid exhales acid in a 60-foot line that is 5 feet wide. Each creature in that line must make a DC 17 Dexterity saving throw, taking 49 (11d8) acid damage on a failed save, or half as much damage on a successful one.
A colossal, wormlike monster from the depths of the earth, the neothelid is a close relative of the illithids, but grows to over 100 feet long and possesses deadly psychic powers. Worshipped and served by many lesser creatures, neothelids once ruled over vast empires, although now many no longer remember them.
