Death Burst. If the pufferfish dies while enlarged, it explodes in a burst of water. Each creature within 20 feet of it must make a DC 12 Dexterity saving throw, taking 16 (3d10) bludgeoning damage on a failed save, or half as much damage on a successful one.

Hold Breath. While out of water, the pufferfish can hold its breath for 30 minutes.

Water Breathing. The pufferfish can only breathe underwater.

Spiked Hide. At the start of each of its turns, the pufferfish deals 5 (1d10) piercing damage and 3 (1d6) poison damage to any creature grappling it.

ACTIONS

Enlarge (Recharges after a Short or Long Rest). For 1 minute, the pufferfish increases in size. While enlarged, the pufferfish is Large, doubles its damage dice on Strength-based weapon attacks (included in the attacks), and makes Strength checks and Strength saving throws with advantage. If the pufferfish lacks the room to become Large, it attains the maximum size possible in the space available.

Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 4) piercing damage or 13 (2d8 + 4) piercing damage while enlarged, and the target is grappled (escape DC 14). Until this grapple ends, the target is restrained, and the pufferfish can't bite another target.

Giant Gulp (Recharge 6). The pufferfish pulls massive amounts of water into its mouth in a 30-foot cone if it is underwater. Each creature in that area must make a DC 12 Dexterity saving throw. On a failed save, a creature takes 5 (1d10) bludgeoning damage or 11 (2d10) bludgeoning damage while enlarged, and the creature is pulled 10 feet closer to the pufferfish. If a creature is moved adjacent to the pufferfish by this, the pufferfish can make one bite attack against a single such target as a bonus action. On a successful save, the creature takes half as much damage and isn't pulled.

REACTIONS

Defensive Offence. When the pufferfish is hit by a melee attack, the attacker must succeed on a DC 12 Dexterity saving throw or take 5 (1d10) piercing damage and 3 (1d6) poison damage.

The giant pufferfish is an aquatic beast that generally keeps to itself, peacefully feeding on schools of fish unless threatened by more dangerous predators, at which point it will enlarge itself by filling up with water, becoming a deadly sphere of poisonous spikes.
