Large monstrosity (festerheart), neutral evil Armor Class 16 (chain mail)
Saving Throws Con +10, Wis +7 Poisonous Body. A creature that touches the festerheart or hits it with a melee attack while within 5 feet of it takes 10 (3d6) poison damage. Restorative Hunger. For every pound of food the festerheart eats, it regains 5 hit points. ACTIONSMultiattack. The festerheart makes three attacks, only one of which can be with its bite. Bite. Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit: 12 (2d6 + 5) piercing damage and 10 (3d6) poison damage and the festerheart regains hit points equal to the damage dealt. Greataxe. Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit: 18 (2d12 + 5) slashing damage and 10 (3d6) poison damage. Javelin. Melee or Ranged Weapon Attack: +10 to hit, reach 5 ft. or range 30/120 ft., one target. Hit: 12 (2d6 + 5) piercing damage and 10 (3d6) poison damage. Gory Feast. The festerheart feeds on the corpse of one creature within reach that died within the past minute. The festerheart regains 9 (2d8) hit points and gains advantage on Strength-based attack rolls, Strength checks, and Strength saving throws for 1 minute. Leadership (Recharges after a Short or Long Rest). For 1 minute, the festerheart can utter a special command or warning whenever a nonhostile creature that it can see within 30 feet of it makes an attack roll or a saving throw. The creature can add a d4 to its roll provided it can hear and understand the festerheart. A creature can benefit from only one Leadership die at a time. The effect ends if the festerheart is incapacitated.

The leaders of the festerhearts are the festerheart mayors, transformed by the Festering into hulking behemoths. They ooze foul liquid from every pore, which they cover their weapons with as a deadly poison or simply allow it to wash over those enemies who draw too near. |
