Relentless (1/Day). If the eikalesh takes 66 damage or less that would reduce it to 0 hit points, it is reduced to 2 hit points instead.

Legendary Resistance (5/Day). If the eikalesh fails a saving throw, it can choose to succeed instead, unless the failure is by 10 or more.

ACTIONS

Bite. Melee Weapon Attack: +18 to hit, reach 20 ft., one target. Hit: 37 (6d8 + 10) piercing damage plus 22 (4d10) poison damage, and the target is swallowed if it fails a DC 26 Strength saving throw. While swallowed, a creature is blinded and restrained, has total cover against all effects originating outside the eikalesh, and takes 55 (10d10) acid damage at the start of each of the eikalesh's turns. The eikalesh has six heads, each of which can swallow two creatures.
If the eikalesh takes 50 or more damage on a single turn from a creature inside it, it must succeed on a DC 28 Constitution saving throw or regurgitate the creature which falls prone within 30 feet of the eikalesh. If the eikalesh dies, a swalowed creature is no longer restrained by it, and can escape the corpse using an action and 10 feet of movement, exiting prone.


The eikalesh is a gigantic, many-headed creature with immense strength and even some psychic abilities. An eikalesh is 25 feet tall and 45 feet long, including a 20-foot-long tail. It can also blast enemies with its mental abilities. But its worst weapon is its devastating ability to swallow creatures.
