This appears to be some sort of light-weight crossbow that straps to your wrist, allowing it to be fired on your arm but still holding another weapon.

This weapon straps on your arm and can be fired that like a hand crossbow. A small string is attached to your finger and tied via pully system to the firing mechanism. It is a free action that provokes an attack of opportunity to reload the crossbow, and you can fire, and load it with one hand. You cannot use another weapon with the wrist crossbow, however, you can hold an item in that hand while firing. If used in your off-hand, you suffer a -2 penalty to all attack rolls with that arm. You can mount the wrist mounted crossbow on the underside of the forearm and conceal it with a sleight of hand bonus of +2, but you also take an additional -2 to attack rolls while using it mounted in this position.
