The arms of the horrid one are a pair of severed arms once belonging to a creature of an indeterminable race, preserved through an unknown alchemical process so they don't decay. They can be grafted onto a willing or incapacitated humanoid creature with a successful DC 20 medicine check, following 1 hour of surgery. On an ordinary failure, the creature suffers unpleasant complications from the surgery; its hit point maximum is halved for 30 days. On a failure by 5 or more, the creature dies on the operating table. Except on a failure by 5 or more, the creature gains an additional pair of arms, which are now considered a permanent part of the creature's body.

Once the arms of the horrid one are grafted onto you, you gain the following benefits:

  • The arms of the horrid one are a fully functional pair of arms. They can be used to attack, benefit from a shield, or perform the somatic components of a spell, even if your natural arms are full or otherwise occupied. You gain an additional action on each of your turns, which you can only use to take the Use an Object Action.
  • You can engage in two-weapon fighting, even if the melee weapons you're wielding aren't light or one-handed.
